Dear client, a gentle reminder of our most important terms and conditions that has been already approved and by which Everlast center offers its medical services to its clients:

– Kindly be informed that the treatment cannot be transferred to friends or family members as this is a part of the patient’s treatment plan and transferring these services without completing the treatment plan as recommended by the doctor may lead to unsatisfactory results 

– Kindly be informed that the package purchased is non-refundable, non-transferable., the amount paid can be considered as an advance payment for any other treatment with the full amount only in case the treatment purchased sessions hasn’t started.

– Kindly be informed that the cancellation of an appointment for this offer is only accepted if the appointment is changed 4 days prior to the booking date for one time only and is done based on the available appointments.

– Kindly be informed that not showing up after confirming the appointment, will result in the loss of one of your treatment sessions from the package purchased and it can’t be replaced or rescheduled.

– Kindly be informed that a package financial validity is a year from the purchase date but once the treatment has begun, the medical validity is according to the table below

* Skin care & facial packages are valid for 4 months (1 session every 4 weeks)

* Body packages valid for 2 months (1 session every week)

* Rejuvenation packages (PRP, derma pen, Pico/ fractional, meso, RF)
valid for 6 months (1 session every month or as per DR plan)

* Laser packages valid for 1 year (1 session every 4 to 8 weeks max)

 I agree to receive medical treatment and care at Everlast Wellness Medical Center, and I agree to accept the services that the Centre provides to me, including services that diagnose my medical condition, as well as the medical procedures that the medical staff of this Centre are doing to treat my condition.

 

• I know very well and I am fully aware that these services will be provided to me by doctors, nurses, physician assistants and others from the medical staff, and I do not mind that some trained members of the medical staff participate in my care under the supervision of specialists. I agree to this of my own free will without coercion, fraud, fraud, ignorance or deception, and this is my acknowledgment of that.

• I know very well, without any ambiguity or confusion, that my consent falls within the framework of the general approval of the services provided to me by the Everlast Wellness Medical Center, which include customary medical procedures and treatments such as drawing a blood sample, physical examination, administering medications, imaging, local anesthesia and any other procedures not classified in List of common actions and I agree to it.

• I do acknowledge that different declarations may be needed for some specific diagnostic and surgical procedures and I agree to it.

• I know very well and understand that Services provided by the medical center are listed under medical treatments, thus cannot be refunded and I agree to it.

• I know well and understand that I must benefit from any treatment plan presented to me in the form of packed sessions in a period of time not exceeding 12 months according to the guidelines of treatment protocols, and in the event that these sessions are not completed, the responsibility falls entirely on me without the slightest responsibility on the shoulders of the center. including, Probably that The patient is not allowed to recover the value paid. and I agree to it.

• I know very well, and I understand that any request to exchange packed therapy sessions should be after a medical consultation to determine the effectiveness of the new treatment, and I agree to it.

• I know very well, and I understand that Exchanges are done based on original prices, if treatment packages were purchased during a seasonal offer/discount which is no longer active. and I agree to it.

• It is known to me that the results of medical and surgical treatments cannot be accurately estimated in advance, and that the treating medical center or staff will not provide me with any guarantees or assurances regarding the results of care and treatment. Accordingly, the center or medical staff is not responsible for the satisfactory medical results for me personally and I agree to it.

• I give permission to review my medical data upon request from the Health Authority – Abu Dhabi, and the approved bodies and the relevant insurance company in accordance with the laws and regulations of the Health Authority., while maintaining complete confidentiality and privacy according to the laws and rules approved by the Health Authority..

• I grant permission and I agree for my medical data to be used for clinical research, if needed, with the understanding that my identity shall remain confidential and privacy respected.

• I authorize and consent for taking photograph/s using Vectra/ Next Motion Device before and after the treatment/procedure to document baseline and to assess, record and monitor progress.

• I authorize and agree to perform a digital skin analysis to obtain a clear picture of the current condition of my skin and to create a customized treatment plan tailored to my medical condition.

• I know very well that my consent to these services will remain valid until either the end of my treatment or that I personally request to stop the treatment and accordingly the center does not bear the slightest responsibility..
• I know very well that the Everlast Wellness Medical Center is the only one that manages the process of organizing reservations and has the right to the optimal choice of reservation with the doctor who works for him and what it includes of changing reservations and changing his schedule and changing the chosen doctor in the event of his unavailability or vacancy, and I agree to that
• I take full responsibility for all my personal belongings, including but not limited to personal glasses, hearing devices, dental prostheses, jewelry, money and other valuables. I know that valuables can be deposited in the center’s safe upon my request, and other than that, the center does not bear the slightest responsibility except for the property placed in its safe.
• I pledge to pay the material costs that may result from examinations and treatment in accordance with the laws and regulations followed in the United Arab Emirates. In the event of non-payment, the center has the right to stop providing treatment or medical services.
• This acknowledgment is valid as long as I am still under treatment at Everlast Wellness Medical Center.
• In case of refusal please fill up the specific Refusal Form.
• I have been informed of and agree to all my rights and responsibilities.

I declare that I have read, understood and agree to the above information and items in its entirety without division, and therefore I sign to it of my own volition.
